HIPAA Authorization

HIPAA is a federal law which is also referred to as the "Privacy in Medicine Law." Basically, HIPAA restricts a doctor or hospital from giving information to people concerning you, the patient.
Normally this is good, as you do not want your private medical information spread to the wrong people. However, it is very possible that you would like medical information going to certain of your family members or friends. It is equally as possible that the medical providers will not be allowed, under the law, to release your information to people you feel need the information.
To remedy this situation, we provide a HIPAA Authorization as part of the Estate Planning documents we provide. This form allows anyone you select to have the opportunity to review your medical information so they will be informed of your situation and be able to offer help as it is needed.
