Healthcare Power of Attorney - This is a document valid in most States which allows you, while you are competent, to choose someone you love and trust to make medical decisions for you. These conditions are usually after you have been examined by more than one doctor, and all the doctors determine to a medical certainty that you are either permanently unconscious or you have a terminal condition and death will result in a reasonably short period of time.
